These documents (3 months limit after end of policy) :
1- Your policy number.
2- All medical invoices, prescriptions.
3- The original "feuilles de soins" with each medicine's stickers.
For all surgery/hospital/radiography : request our medical certificate to be
filled-in by your doctor.
